Client Care Worker (Per Diem)

Requirements
Either: (a) High
school diploma and 1 yrs work experience, preferably in human services
delivery, (b) 5 yrs work experience in public or private health or social
agency, or (c) satisfactory equivalent combination. Must be familiar with
issues of HIV/AIDS, substance abuse, and mental health; and be able to work
effectively with a poly-diagnosed population. NYS driver's license required.
Job Description
The Client Care
Worker assists the Client Care Supervisor in assuring the safe, secure,
comfortable and effective operation of the Shelter program. This Person is
responsible for providing guests with the necessities of daily living and
familiarizing them with program policies, rules, and expectations.
Specifically, the CCW: (a) assigns guests lockers and distributes
toiletries; (b) monitors individuals who come to and leave the building; (c)
consults with the Client Care Supervisor regarding observations of unusual
client behavior; (d) assists guests at mealtime; (e) assists in the
assignment of beds, and distribution of linen and snacks; and, (f) assists
guests with matters of personal hygiene.

Site Monitor Broadway

Requirements

diploma and 2 year's work experience in human services delivery; (b) GED and
3 years experience; or, (c) a satisfactory equivalent combination. Must be
familiar with issues of HIV/AIDS, substance abuse, and mental health, and
have good written and oral communication skills. A mechanical/engineering
aptitude helpful. Must be able to work effectively with a poly-diagnosed
population. NYS driver's license required.
Job Description
The Site Monitor assists the
Site Supervisor in assuring the security, repair and cleanliness of the
building, its equipment and systems, and the comfort and security of its
occupants. She/he monitors access and egress, negotiates interpersonal
disputes, arranges the serving of meals, administers occupant mail and
messages, and all other activities of daily living. She/he intervenes in
emergency situations.

H.O.S.T
Worker (Per Diem)
Job
Requirements
Either: (a) Bachelor's
degree and 1 yr experience in human service delivery, (b) high school
diploma and 3 yrs experience, or (c) satisfactory equivalent combination.
Must possess strong knowledge of HIV/AIDS, substance abuse, SSI/D
entitlements, and mental health issues; have good oral and written skills,
and be able to work effectively with a poly-diagnosed population. Valid NYS
driver's license required. Bi-lingual (English/Spanish) a plus.
Job Description
Working from a
specially-equipped van from The Sharing Community's facilities, the H.O.S.T
Worker for the HOST (Homeless Outreach & Service Team) project will travel
an established geographical route to engage unsheltered homeless individuals
and others in need. The program operates 7 days/week, from 6 a.m. to 10
p.m., to provide access to social services and local community resources,
direct case management, advocacy in obtaining entitlements, and/or temporary
emergency residence for homeless individuals.

Requirements
The minimum requirements for the position is as
follows: Must satisfy the following criteria: (a) Master’s
degree in Psychology, Social Work, Public Health or other clinical
discipline and 2 year experience in HIV issues, mental health/substance
abuse treatment or public health. (b) Bachelor Degree in any of above
disciplines and 3 yrs experiences or (c) Satisfactory equivalent to be
determined by appropriate director. Prior supervisory or management
experience required. Strong knowledge of issues around HIV/AIDS and Public
Health. Local and out-of-state travel required. Bi-lingual (Spanish/English)
a plus. Must be computer literate. Knowledge of AIRS a plus.
NYS driver’s license required.
Job Description
The Project
Coordinator is responsible for the effective street and community outreach,
risk reduction education, and linkage of high risk targeted individuals to
all essential services. This includes supervision of staff and personnel
management, coordination with various outreach programs and other program
components and community resources, identification of training needs and
opportunities, monitor structured evidence-based interventions, review of
documentation and relevant data entry, team building and leadership and
program development. The Project Coordinator will also conduct program
evaluation and produce monthly, quarterly, and annual reports to funders and
recommendations for modification of program budget as needed and supervise
data entry by assigned staff.
